Existing prediction models, largely derived from Caucasian cohorts, fall short in accuracy for India’s ethnically diverse population, shaped by unique genetic, dietary, and lifestyle factors. The integration of multi-omics data with artificial intelligence-driven analytics heralds a paradigm shift towards personalised, predictive, preventive, and participatory (4P) medicine.The Phenome India-CSIR Health Cohort Knowledgebase (PI-CHECK), a flagship CSIR initiative, establishes a Pan-India longitudinal cohort of employees, pensioners, and spouses, leveraging CSIR’s nationwide laboratories for assessments at three time points over a five-year period. It integrates multimodal data clinical, anthropometric, biochemical, imaging, and molecular (genomics, proteomics, metabolomics, microbiome)-analysed via AI/ML to deliver precise, India-centric risk models.Launched on 7 December 2023, PI-CHECK enrolled more than 10,000 participants. Data collection, facilitated through a secure OTP-authenticated portal, includes rapid sample processing and report dissemination within 48-72 hours using portable diagnostic tools.Phase 1 concluded with 10,261 participants, celebrated at Phenome India: Unboxing 1.0 on 3 June 2024 at CSIR-NIO, Goa. Phase 2 sample collection wrapped up in October 2025, with Cohort Connect scheduled for 13-14 November 2025 at CSIR-IMMT, Bhubaneswar, coinciding with World Diabetes Day.PI-CHECK pioneers representative health profiling while championing inter-cohort connectivity.
